How to Register:

Please register by completing the following on-line form. (You may also register by mail by sending the information requested below with a check to the address listed at the bottom.) Please complete a separate on-line form for each person attending, so we are able to keep an accurate count and streamline data entry. If you make a significant error, just fill out the form again and add a comment, as it is easy for us to eliminate duplicates. Items with an asterisk are required fields. Please enter n/a if not applicable or if you would simply rather not supply the information.

A registration confirmation will be automatically emailed to you after you complete this form, although your registration will not be considered to be complete until payment is received. Please be sure that your email address is entered correctly!
