International Student Sponsorship Forms

International students who apply to the master of divinity or the master of arts programs at Bethany are required to submit the following forms:

• Statement of Personal Sponsorship – to be completed by a church or agency
• Financial Sponsorship Form – to be completed by those offering financial support for the applicant
• Financial Worksheet of projected expenses – to be completed by the applicant

These forms are available at the link below. Please print them out, arrange for the appropriate parties to complete them, and return them to us as part of your application.
